Nomination Guidelines and Forms

All Nomination Packages MUST Include:

  1. āœ… A Nomination Form.

  2. āœ… SBA Background Form 3300Ā – For ā€œteamā€ nominations, a background form is required for each team member.
  3. āœ… SBA Authorization, Grant, & Release of Materials FormĀ – For ā€œteamā€ nominations, a release form is required for each team member.
  4. āœ… Photo – An original 8ā€ x 10ā€ or 5ā€ x 7ā€ photo of the nominee or a digital photo ā€“ at least 300 dpi isĀ required; photocopies are not acceptable. For ā€œteamā€ nominations, a photo of eachĀ nominee or group photo is acceptable.
  5. āœ… Additional Supporting DocumentationĀ – deemed significant by the nominator, includingĀ news clips, letters of recommendation, nomination letter (if not self-nominated), andĀ other evidence of the appropriateness of the nomination. Supporting documentation mayĀ not exceed 10 pages. Videos will not be considered.
